Copay only — no additional fees
Other Commercial Insurance
Assessment fee: $75
Self-Pay / Cash Rate
Assessment fee: $100
Urinalysis (U/A) is included in the cash rate.
Insurance We Do Not Accept
We are unable to bill the following insurance types:
Obamacare / ACA Marketplace Plans
Medicare
Tricare
Other Government Insurance Programs
